Currently in order to be eligible for our services individuals must meet the two following Criteria:

  • Children with Serious Emotional Disorders (see SED checklist)
  • Have a diagnosis for a Mental Health Issue or Substance Use Disorder.
  • Be 4-18 years old.
  • Reside in Bernalillo County.

Program Description

The child CCSS team provides case management/community support services to children, youth, and their guardians. Referrals can be submitted directly by providers at UNMH for any individual experiencing serious emotional disturbances whose functioning limits their ability in some or all life domains. Community Support Workers on this team can meet in a range of settings including schools, homes or other community settings. Community Support Workers coach individuals through learning skills and navigating services, resources, and benefits, with the hope of eventual graduation

Services Provided

  • Comprehensive case management for children age 4-18 and their guardians.
  • Safety and treatment planning.
  • Support in maintaining and accessing mental health services
  • Support with accessing medical providers and community resources
  • Assistance navigating the education system, social services, benefits and assistance programs.
  • Assistance accessing vital documents
  • Education on utilizing coping skills
  • Program is voluntary. Individuals must be willing
  • to engage with Community Support Workers.
