Program Description

The TPT provide transition planning services for those incarcerated and leaving custody at the Metropolitan Detention Center, with a special focus on those who are in higher risk categories and more likely to recidivate. This team supports reentry to the community through behavioral health service and resource connection, hoping to provide a clear plan, based on self-identified goals, for immediate next steps following release.

Services Provided

  • Connect clients to Mental Health Services in the community.
  • Refer clients to Mental Health and Substance Abuse Programs in the community.
  • Collaborate with Providers to determine appropriate placement.
  • Assist in obtaining a treatment guardian.
  • Educate on resources available in the community.
  • Coordinate appointments prior to release.
  • Facilitate interview interviews while in custody.
  • Resource management and system navigation skills
  • Assist in communicating concerns to Medical and Psychiatric Providers.
  • Complete Certificates of Evaluation.

Referral process

When individuals are booked into MDC they complete a set of screener meant to measure an individuals risk to recidivate. Individuals that are found to be at a high risk to recidivate (6-8) are assigned a Transition Planner.
