Program Description
The CCSS team provides case management/community support services to clients who are followed by psychiatric providers at UNMH Clinics. Referrals can be submitted directly by providers at UNMH for any individual experiencing severe mental illness whose functioning limits their ability in any or all of life domains. Community Support Workers on this team coach individuals through learning the skills necessary to build greater independence in navigating services, resources, and benefits, with the hope of eventual graduation.
Services Provided
- Help the client define what recovery means to them and help them develop SMART goals -
- Specific, Measurable, Attainable, Relevant,
- and Timely.
- Help the client identify and recognize symptoms of a potential crisis situation and
- early signs of relapse (relapse encompasses
- any type of regression).
- Assist in developing interpersonal and functional skills.
- Assist in developing and/or expanding the client's support system.
- Assist in building symptom management skills including behavior activation.
- Help educate clients about their diagnoses, medications and their side effects.
- Assist clients link to resources in the community, including medical providers.
- Assist with practical and vocational skills such as budgeting, maintaining benefits, housing,
- and employment.
- Monitor progress to determine how services are meeting the clients' needs.
