Refer a Patient

At UNM Gastroenterology and Hepatology, we are committed to making the referral process as simple and efficient as possible for you and your patients. Our team welcomes referrals from primary care providers, specialists, and other healthcare professionals across New Mexico and the surrounding region.

Once we receive a completed referral form, our scheduling team will contact your patient promptly to arrange their appointment or procedure.

How to Refer

Complete the appropriate referral form below, and fax it to us on the number listed. A member of our staff will follow up with your patient within 1-2 weeks to schedule their visit.

For urgent referrals or complex cases requiring immediate attention, please call us directly at (505) 925-6000.

Step 1

Download, complete, and fax the form that best matches your patient's needs:

General GI Procedures: For patients requiring routine diagnostic or therapeutic endoscopic procedures, including colonoscopy, upper endoscopy (EGD), flexible sigmoidoscopy, capsule endoscopy, feeding tubes etc.

Step 2

Advanced GI Procedures For patients requiring complex or interventional endoscopic procedures, including ERCP, endoscopic ultrasound (EUS), ESD, EMR, POEM, GPOEM, RFA/ablation, balloon-assisted enteroscopy, enteral stent placement, EUS-guided drainage, TIF, Zenker's diverticulotomy, celiac plexus block, endoluminal suturing etc. Please include relevant imaging reports, prior procedure notes, and laboratory results with this referral.

Step 3

Clinic Appointment or Special Procedures For patients requiring a new outpatient consultation, follow-up visit, or evaluation for a specific GI or liver condition — including IBD, liver disease, motility disorders, eosinophilic esophagitis, and other complex diagnoses. Also use this form for breath testing, esophageal and anorectal manometry, and pH monitoring or Bravo referrals.

What to Include With Your Referral

To help us prepare for your patient's visit and minimize delays, please include the following with your referral form where applicable:

  • Relevant clinical notes and history
  • Recent laboratory results (CBC, CMP, liver function tests, etc.)
  • Prior imaging reports (CT, MRI, ultrasound)
  • Prior endoscopy reports and pathology results
  • Current medication list
  • Patient insurance information and photo ID
