Anticoagulation Clinic
The UNM Anticoagulation Clinic is a pharmacist-run clinic focused on blood thinner medication. We are an Anticoagulation Center of Excellence.
If you have been prescribed Warfarin (Coumadin), Apixaban (Eliquis) or Rivaroxaban (Xarelto), ask your doctor for a referral to our clinic.
Our pharmacists will help you receive safe follow-up on your medication. We provide education and counseling, as well as:
- Routine monitoring and dosage adjustment
- Peri-procedural management
- Risk/benefits for continued anticoagulation usage
We now also provide education and counseling for the new anticoagulants known as direct-acting oral anticoagulants (DOAC). We assist with the switch to and from DOACs and warfarin. We serve as a help center for managing your new medication.

Our new, permanent location is at 1209 University Blvd. NE, Albuquerque, NM 87102. (cross-streets: University Blvd./Camino de Salud). Access to the clinic is through the WEST entrance of the building. Look for ‘Pharmacy Clinical Services’ on the outside of the building.
We are open 8:00am to 4:00pm, Monday through Friday

As part of UNMH's Antithrombosis Services, which also includes the inpatient antithrombosis team and the outpatient anticoagulation clinic, we are recognized by the Anticoagulation Forum’s assessment program.
