Anticoagulation Clinic

The UNM Anticoagulation Clinic is a pharmacist-run clinic focused on blood thinner medication. We are an Anticoagulation Center of Excellence.

If you have been prescribed Warfarin (Coumadin), Apixaban (Eliquis) or Rivaroxaban (Xarelto), ask your doctor for a referral to our clinic.

Our pharmacists will help you receive safe follow-up on your medication. We provide education and counseling, as well as:

  • Routine monitoring and dosage adjustment
  • Peri-procedural management
  • Risk/benefits for continued anticoagulation usage

We now also provide education and counseling for the new anticoagulants known as direct-acting oral anticoagulants (DOAC). We assist with the switch to and from DOACs and warfarin. We serve as a help center for managing your new medication.

About Us

As part of UNMH's Antithrombosis Services, which also includes the inpatient antithrombosis team and the outpatient anticoagulation clinic, we are recognized by the Anticoagulation Forum’s assessment program.

Participation in this program demonstrates our ongoing goal to achieve the highest level of care and the best possible outcomes for our patients on antithrombotic medications.
