Individuals in MDC custody who are housed in the Psychiatric Units.
Program Description
Discharge Planners provide connection to services, treatment, and resources for those incarcerated and leaving custody at the Metropolitan Detention Center, with a special focus on those who have serious medical and/or psychiatric, and/or Medically Assisted Treatment needs, providing intensive case management. Offsites Coordinators connect individuals to ER or specialty clinic care for those in custody.
Services Provided
- Connect clients to Mental Health Services in the community
- Refer clients to Mental Health and Substance Abuse Programs in the community
- Collaborate with Providers to determine appropriate placement
- Assist in obtaining a treatment guardian
- Educate on resources available in the community
- Coordinate appointments prior to release
- Facilitate interview interviews while in custody
- Resource management and system navigation skills
- Assist in communicating concerns to Medical and Psychiatric Providers
- Complete Certificates of Evaluation
Referral process
Individuals housed in any of MDC's Psychiatric Units is assigned to a Discharge Planner. Each Psychiatric Unit has a Discharge Planner assigned to that Unit. Individuals are placed in a Psychiatric Unit based on the assessments completed by the MDC Psychiatry providers.
