Currently in order to be eligible for our services individuals must meet the two following Criteria:
- Be involved in the criminal justice system OR be at risk of becoming involved.
- Have a diagnosis for a Mental Health Issue or substance use disorder.
- Be 18 years or older.
Program Description
JDS provides comprehensive case management and community support services to clients with severe mental illness (SMI) and criminal justice involvement. Clients are most often referred by the criminal justice system. This team works closely with Metropolitan Detention Center, District/Metro Courts, state and federal probation and pre-trial services on referrals. The programs of Fast Track and Forensic Navigation fall under JDS, as well as the Justice Involved agreement with BHSD. Individuals served are supported in accessing a variety of resources and benefits to enhance quality of life in all life domains, including independent living, learning, working, socializing, and recreation. Psychiatric and substance abuse services, as well as support with legal navigation.
Services Provided
- We provide comprehensive case management.
- We assist clients in creating a treatment plan and identifying their goals for release
- We connect clients with mental health and substance treatment providers
- We can help coordinate medication upon release We assist clients in securing space at shelters such as Good Shepherd, AOC, Joy Junction etc. We help clients obtain their birth certificates, social security cards, IDs
- We provide tools for clients to learn how to develop life skills and coping skills
- We assist clients in following up with their pre-trial
- and/or probation officers, and attend any subsequent court hearings
Program is voluntary. Individuals must be willing to engage with Community Support Workers.
