4ACC Outpatient & Discharge Pharmacy

Located on the 4th floor of the hospital, the 4ACC Outpatient & Discharge Pharmacy provides convenient pharmacy services for hospital patients, including patients preparing to leave the hospital and patients who visit the pharmacy for their outpatient prescription needs.

Our pharmacy team works closely with patients and their healthcare providers to help make sure medications are ready, affordable, and understood before patients leave the hospital.

Meds to Beds

Leaving the hospital with your medications in hand can make the transition to home safer and easier. Our Meds to Beds service allows eligible patients to have their discharge prescriptions filled by the UNMH Outpatient & Discharge Pharmacy and delivered to the patient before they leave the hospital.

Without their medications, patients may face delays in starting treatment or difficulty getting prescriptions filled after discharge. Having medications ready before leaving the hospital can help patients begin their treatment plan as instructed and reduce barriers to obtaining their prescriptions.

Our pharmacy team works with the patient's healthcare team and insurance provider to help address prescription and coverage needs before discharge.

The 4ACC Outpatient & Discharge Pharmacy offers a range of services to help patients access and manage their medications, including:

• Medication counseling: Our pharmacists provide education about new and existing medications, including how to take them, what to expect, and what to watch for.
• Meds to Beds: Eligible patients can have discharge prescriptions filled and delivered before leaving the hospital.
• Prior authorization support: We work with your healthcare provider and insurance company when your medication requires additional approval.
• Refill authorization: When a prescription requires provider approval for a refill, our pharmacy team can contact the appropriate healthcare provider.
• Medication access support: We help identify and address barriers that may prevent patients from obtaining their medications.
• Affordable medication options: Our pharmacy team can work with healthcare providers to identify alternative therapies or medication options when appropriate and when cost is a concern.
• Prescription processing: We only fill outpatient prescriptions and discharge medications prescribed by a UNMH healthcare team.
• Medication questions: Pharmacists are available to answer questions about your medications and help you understand your treatment.
• Care coordination: Our pharmacy team communicates with healthcare providers to help coordinate medication-related needs and support continuity of care after discharge.

• Patients being discharged from UNM Hospital
• Patients being seen at UNMH Urgent Care or Emergency Department
• Patients with UNMH prescriptions
• Even if you are not being discharged from the hospital, we can still fill your UNMH prescriptions on a walk-in basis.
