Program Description
This team provides intensive case management services to clients with a severe mental illness (SMI) diagnosis and/or co-occurring disorders, and a history of justice involvement who are receiving a housing voucher from Bernalillo County Community Connections Supportive Housing. This team is contracted with the county to provide intensive case management services to those in the housing program. Referrals may only come directly from the County managed Community Connections voucher wait list for screening into the program. All life domains can be addressed in terms of service and community resource connection.
Services Provided
Individuals served are supported in locating and maintaining stable housing, accessing a variety of community resources and benefits to enhance quality of life in all life domains, including the following: independent living, learning, working, socializing, and recreation, psychiatric and substance abuse services access, as well as support with navigation of the justice system when applicable.
