Ages 18 and above with severe mental health diagnosis.

Program Description

The ACT team provides therapy, case management, peer support services, medication management, psychiatric nursing, and rehabilitation and support services for people living with severe mental illness, often experiencing comorbid diagnoses. Individuals best served by this intensive service are those who have not been successful in maintaining consistency with care in clinic settings or responded well to traditional outpatient care, those without personality disorder diagnoses, and without frequent expression of violence episodes. Referrals come from a universal application which feeds City of ABQ ACT teams.

FACT offers the same services as mentioned above for those who are Justice Involved 

Services Provided

  • Wraparound intensive mental health services in the community 2-3 times a week on average.
  • Medication management and support.
  • Individual counseling.
  • Group activities.
  • Crisis intervention with 24/7 on call service to ACT clients.
  • Case management.
  • Peer support.
  • Vocational rehabilitation.
