Request Medical Records

To request a copy of your UNM Health System medical records, fill out a form in English [PDF], Spanish [PDF] or Vietnamese [PDF]. This gives us permission to share your health information. You must fill out all fields and initial where requested.

You can also view, download, and share parts of your UNM health using MyHealthUNM, UNM’s patient portal. For help creating a new account, call 505-272-2141.

UNM Hospital Medical Records

The medical records office is located on the first floor of the main hospital. UNM Hospital can release records for UNM Hospital, UNM Sandoval Regional Medical Center (SRMC) and the UNM Comprehensive Cancer Center.

UNM Sandoval Regional Medical Center Records

The medical records office is located at SRMC, and can release records for SRMC, UNM Hospital and the UNM Comprehensive Cancer Center.

Fees for SRMC Records and Images

You may be charged to receive the following medical records and images:

  • Central processing fee that includes requests for third-party use, such as attorneys and record copy service agents:
    • Base fee of $20 for up to 15 pages when hard-copy file is required
    • $1 per page for more than 15 pages
    • Base fee of $1 if patient information can be printed from Power Chart
    • $10 rush fee if records are needed within three working days
  • Subpoenas with affidavit: $10 extra
  • Viewing/sitting fee: $20 per hour
  • Workers’ compensation attorney charges, mandated by state law:
    • $10 for the first 10 pages
    • $0.20 per page for every page over 10
  • Disability determination services: $18.75 minimum/maximum
  • Department of Vocational Rehabilitation: $26 minimum/maximum
  • Patients: $0.11 per page
  • Images on CD: $10 per CD

There are no charges for:

  • Job Corp, physicians, schools, therapists, health clinics, hospitals (nonprofit organizations for continuum of care)
  • Shot records/vaccinations
  • Social Security insurance

UNM Comprehensive Cancer Center Records

In accordance with HIPAA regulations, you may request copies of your medical records for personal use. Contact our medical records office at 505-925-0106 for more information.

Health Information Exchange

A Health Information Exchange (HIE) is an electronic system which allows your care team to share and view healthcare information between other doctors, hospitals or clinics.

  • New Mexico has several Health Information Exchanges.
  • HIEs can give your care team a better picture of your overall healthcare.

Federal and state laws allow and sometimes require health systems to share information to HIEs automatically. However, you can sometimes refuse to share your information to a HIE.

Besides having some choice about sharing your information, you also control whether we view your information in HIEs.

  • You can allow us to view your information in HIEs during any outpatient, inpatient or emergency visit by completing a consent form in English [PDF], Spanish [PDF] or Vietnamese [PDF].
  • If you refuse to allow us to view your information in HIEs, we will not, unless it is allowed as an exception by state law.

Please remember HIEs are intended to give your care team at UNM more information about your overall health, which allows us to provide better care for you.
