Hospital Affiliation Requests
The Office of Medical Staff Affairs is pleased to provide you with a fast and accurate on-line verification service. You can obtain an instant verification of Medical Staff or Allied Health Staff affiliations for any of the practitioners associated with the University of New Mexico Hospitals or UNM Sandoval Regional Medical Center through the NAMSS PASS centralized data repository.
Please go access the NAMSS PASS database. There is a small fee for each individual affiliation letter download.
Each UNM Health System entity is responsible for verification of entity-specific affiliation and privileges. Each request should be submitted in writing, with authorization/consent from the practitioner to:
